Complex & Clinical Care - The Diamond of Excellence
Advanced Clinical Care at Home
The Diamond of Excellence represents the pinnacle of our clinical capabilities – delivering hospital-level care in the comfort of home. When someone has significant medical or nursing needs, our expert team steps in to provide complex care with precision, skill, and unwavering attention to quality. This service is led by registered nurses and supported by specially trained carers, ensuring that even the most challenging health needs are met safely at home.
Our Diamond of Excellence clients include children and adults with serious medical conditions, people recovering from major surgery or illness, and individuals who rely on medical technology (like ventilators or feeding tubes) daily. We specialise in hard-to-fill placements – cases that require advanced skills or have unique challenges – meaning we won’t shy away from helping where others might not have the expertise. Every care plan is developed and overseen by a senior nurse, in consultation with the client’s doctors and therapists as needed.

Services We Provide
Diamond of Excellence covers what would be considered complex home nursing or paediatric complex care services. We work closely with HSE case managers and hospital discharge planners to coordinate care and ensure everything is in place – from medical supplies to emergency protocols. Our team maintains meticulous records and follows all HIQA guidelines for clinical care, so healthcare commissioners can trust in the quality and safety of our service.
Skilled Nursing Care
Clinical tasks such as administering injections or IV therapies, managing complex medication regimens, catheter and stoma care, wound care and dressings, and monitoring vital signs or symptoms.
Respiratory & Ventilator Support
Caring for individuals with tracheostomies or on ventilators, including airway suctioning, trach changes, oxygen therapy, nebulizer treatments, and close monitoring of breathing.
Tube Feeding & Nutritional Support
Managing PEG or NG feeding tubes for clients who cannot eat by mouth, ensuring proper nutrition and hydration under medical guidance.
Seizure & Medication Management
Support for those with conditions like severe epilepsy, including emergency medication administration if needed and careful observation for safety.
Post-Hospital Transitional Care
After a hospital discharge, providing intensive support at home – for example, wound care after surgery, physiotherapy exercises under guidance, or rehabilitation support to regain strength.
Paediatric Complex Care
Specialised in-home nursing for children with complex medical needs – for instance, infants with congenital conditions, children with severe cerebral palsy, or those requiring long-term ventilation. Our paediatric nurses not only care for the child but also support and educate the family to feel confident in their child’s care routines.
"The Diamond of Excellence Plan brings hospital-level care home, combining clinical expertise with compassionate support. With advanced skills and unwavering dedication, we provide the highest standard of care—because excellence is the foundation of every life we touch."

Our Approach
Excellence is our hallmark. This means we emphasise clinical competence, continuous training, and careful attention to detail. We train our staff in infection control, emergency response, and the specific protocols each client requires. Many of our nurses have ICU or high-dependency unit backgrounds, bringing a wealth of experience. But alongside the technical skills, we never lose sight of compassion – even in complex care, our clients are people first, not just patients. We take time to comfort, to explain what we’re doing, and to ease the stress that medical care at home can sometimes bring.
With Diamond of Excellence care, families can avoid prolonged hospital stays and feel reassured that their loved one’s health is managed by capable hands at home. It truly is advanced care delivered where you need it most. If you have a family member with intensive health needs or are coordinating discharge for a patient who needs ongoing care, our Diamond of Excellence service can provide the support required.
Confidence in Every Detail
When care is complex, precision matters. From medication administration to ventilator checks and wound assessments, our nurses follow rigorous protocols and double-check every step. Families can rest easier knowing that nothing is left to chance — everything is delivered with clinical excellence and full accountability.
Compassion in Complex Care
Even the most skilled procedure can be a source of anxiety for clients and families. That’s why we combine our clinical know-how with emotional intelligence. Whether we’re helping a child through a feeding routine or supporting an adult after major surgery, we take time to reassure, explain, and connect on a human level — because people aren’t just cases, they’re individuals with fears, hopes, and stories.
Specialist Knowledge, Shared with You
Our team includes nurses trained in specialist areas like neurology, paediatrics, and respiratory care. But we don’t keep that knowledge to ourselves — we actively involve families, offering guidance and education so you feel confident in what’s happening at home. We believe that informed families make stronger care partners.
