Specialist & High Dependency Care - The Triangle of Expertise

Advanced Care for Complex Needs & High Dependency Support

The Triangle of Expertise is shaped for people who need more than standard care — those with higher-dependency needs, complex behaviours, or conditions that require specialist training and consistency. This includes individuals with intellectual disabilities, autism spectrum disorders, neurological conditions, and those who benefit from 2:1 support or structured behavioural care.

Each point of the triangle represents a vital part of the plan: the person receiving care, their family or support network, and our expert team — all working in partnership to deliver care that is not only safe and skilled, but calm, respectful, and deeply person-centred.

Services We Provide

Disability services and autism-specific supports, as well as care for those with intellectual disabilities who live at home. We coordinate with HSE disability teams, occupational therapists, and other specialists involved in the client’s care. All care is carefully documented and reviewed by our nurse lead to maintain quality and spot any changes in condition.

Specialist Disability Support

Comprehensive care for individuals with physical or intellectual disabilities. This can involve assistance with personal care, mobility (using hoists, wheelchairs, custom seating), feeding (including understanding of texture modifications or feeding techniques), and communication aids. We focus on maximising the person’s abilities and comfort.

Autism-Specific Care

Caregivers trained in autism support work one-on-one to follow structured routines, visual schedules, and behavioural plans. They help with daily routines, learning life skills, and community outings in a way that respects sensory needs and avoids triggers. Our staff communicates using preferred methods (like picture exchange or assistive technology) to connect with the client.

Intellectual Disability Care

For those with moderate to profound intellectual disabilities, we provide attentive supervision and engagement. This might mean interactive play for a child, skill-building activities for a young adult, or consistent, familiar routines for an older person. Patience and positivity are key – our team celebrates small victories and understands each individual’s unique cues and ways of expression.

High-Dependency Personal Care

Managing the needs of clients who require constant or complex care. This could involve two-person transfers from bed to wheelchair, operating specialised medical equipment (in collaboration with our Diamond of Excellence service if clinical tasks are needed), and ensuring someone is always present for safety. We also handle tasks like seizure monitoring or fall prevention for those at risk, staying alert and prepared.

Behavioural Support & Monitoring

Implementing behaviour support plans for clients who may have aggression, self-harm, or other behaviours that challenge. Our caregivers are trained in de-escalation techniques, positive reinforcement, and maintaining a calm environment. We track any incidents and work with healthcare professionals (like psychologists or behaviour therapists) to continually adapt our approach.

Specialised Respite Care

Enabling family caregivers of high-dependency individuals to take breaks. We can provide short-term cover with staff who already know the client’s specific needs, ensuring consistency of care while the primary caregiver rests or tends to other responsibilities.

"The Triangle of Expertise Plan is built on partnership—connecting individuals, their support networks, and our skilled team to deliver structured, specialised care. With expertise, respect, and consistency, we create a foundation for safety, stability, and dignity."

Our Approach

Expertise and empathy go hand-in-hand. We invest in training our staff in the latest techniques for disability care and autism support. Equally, we value the knowledge of the family – no one knows the person better than their loved ones, so we listen and incorporate family input into our care strategies. We strive for consistency (often assigning a small, dedicated team to each client) because trust and familiarity are crucial in high-dependency care. Our goal is to not only meet the client’s extensive needs but also to provide an enriching life experience – filled with respect, meaningful interaction, and as much independence as possible.

With Triangle of Expertise support, families find relief and partnership. They know their loved one is in capable, understanding hands, receiving the specialised care they require to live safely and fully at home. If you are caring for someone with high-dependency needs or complex disabilities, our Triangle of Expertise services offer the knowledgeable support and relief your family deserves.

Person-Centred Strategies, Delivered with Consistency

People with high-dependency needs thrive in stable, familiar environments. That’s why we place such importance on relationship continuity and person-centred routines. Whether supporting sensory regulation, navigating communication challenges, or implementing behavioural plans, our approach is always tailored, respectful, and informed by best practices in disability and behavioural care.

Specialist Skills, Everyday Empowerment

Our teams are trained in autism awareness, de-escalation, positive behaviour support, manual handling, and assistive technologies. Many also bring experience from disability services, special education, or mental health roles. We blend these clinical and practical skills to enable everyday activities – from hoist transfers and hygiene support to shared mealtimes, social outings, or supported communication.

Supporting Families, Strengthening Care

Families are more than observers – they are central to our care model. We work collaboratively with parents, siblings, and guardians, offering guidance, respite options, and education where needed. We help families navigate challenges and celebrate progress, all while reducing the stress that often comes with managing care alone.

What you’ve just read might be part of the plan — but not the full picture.
Care isn’t one-dimensional. And neither are the people we support.
