Our nurse-led care planning model is guided by our unique framework: the Five Shapes of Care. Each Shape represents a key area of support, and most of our clients benefit from a blend of two or more. This approach ensures care is always:

While we offer a wide variety of services, we organise all care through our unique Five Shapes of Care model — each representing a core type of support. This helps us ensure no part of a person’s needs is overlooked. Every client receives a plan shaped around them — not just their diagnosis, but who they are, how they live, and what matters to them.
01 Circle of Comfort
Personal & Practical Care
Circle of Comfort offers daily living support, including personal hygiene, dressing, bathing, mobility, meal prep, and light housekeeping. Our caregivers provide gentle assistance to help maintain comfort and independence at home.
02 Heart of Compassion
Social Support & Companionship
Heart of Compassion offers companionship and emotional support through friendly interactions, such as chatting, accompanying clients to appointments, or enjoying hobbies together, alleviating loneliness and fostering joy in the home.
03 Star of Dignity
Palliative & End of Life Care
Star of Dignity offers compassionate end-of-life support, specialising in care for individuals with life-limiting conditions. We manage pain and symptoms, ensuring comfort and quality of life, while providing emotional and practical support to clients and their families.
04 Diamond of Excellence
Complex & Clinical Care
Diamond of Excellence provides skilled nursing care for advanced medical needs at home, including medication management, injections, wound care, ventilator and tracheostomy care, tube feeding, and health monitoring for adults and children requiring hospital-level care.
05 Triangle of Expertise
Specialist & High Dependency Care
For high dependency needs, Triangle of Expertise offers services for clients with physical or intellectual disabilities, autism, or other high-support requirements. Our trained team manages challenging behaviours and employs specialised equipment and communication methods, ensuring dignified support at home for those with complex care needs.

Why We Blend Care & What That Really Means
In most cases, a person doesn’t just need one type of support. Real life is more complex — and care should reflect that.
That’s why we don’t just assign services based on a list of tasks. We create blended care plans: carefully considered, nurse-led plans that combine multiple types of support in a way that’s truly integrated.

What Does Blended Care Look Like?
A blended care plan might include:

✓ Clinical nursing such as PEG feeding or epilepsy monitoring (Diamond of Excellence)

Rather than fragment these into separate services — or hand them off between different teams — we blend them into one coordinated experience, delivered by a small, consistent care team, under ongoing nurse supervision.
Why Blended Plans Are Different
Other providers might offer services in “blocks” or “bundles” — one person for personal care, another for nursing tasks, another for activities. But this approach can feel disconnected, rushed, and overwhelming for both the person and their family. Our approach is different:
